In house we use Roundup http://roundup.sf.net/ for issue management and
dokuwiki http://wiki.splitbrain.org/wiki:dokuwiki for internal
documentation and machine log/maint  

I've heard good things about rt but it's overkill for what we want.

No experience with wiki, wonder if that would work.  That rt program
