Check in the registry under:

HKEY_USERS\.DEFAULT\Control Panel\Keyboard (all users)
and/or
HKEY_CURRENT_USER\Control Panel\Keyboard (current user)

Make sure that InitialKeyboardIndicators is set to 0, not 2.

0 = NUMLOCK is turned off after logon, 2 = NUMLOCK is turned on after logon
